Numbered points correspond to the indicators enumerated in the Additional file 1. Blue and yellow points indicate the 59 indicators that achieved above average score for importance and comprise the priority set of rehabilitation indicators. On average, the indicators in the upper right quadrant (blue shaded area/points) achieved above average score for both importance and feasibility and represent the implementation priority set of indicators (Tier 1) whereas indicators in the lower right quadrant (yellow shaded area/points) received below average score for feasibility and comprise the development priority set (Tier 2)
