  1. aNurse staff based on the NHS Workforce Census conducted in September, 2003 by SHA of work (excluding staff of special health authorities or other statutory bodies besides SHAs) [31]. Registered nurse population based on April, 2004 NMC data by postcode of residence mapped to SHA. Vacancy data from the 2004 Vacancy Survey for England [21], ISD Scotland [32] and the Statistical Directorate of the National Assembly for Wales [33]. All figures relate to qualified nurses, midwives and health visitors.

  3. cOver 100% because fewer nurses reside in North Central London than work there (due to nurses who commute from other SHAs).
